Forms and Instructions

The forms listed below, like this:fillable pdf formare Adobe fillable and saveable forms. They can also be printed as a blank form by opening the form and clicking on the print icon. To use the Adobe fill-in forms you need to have the latest version of Adobe Reader. These forms cannot be filed electronically. They can be filled in and saved to your computer. Open the form and use the “Save” button from the Adobe reader to save the completed or blank form to your computer. After completing the form, print, sign and file with the Commission.

  A candidate in one of the three selected districts must file this form by September 28, 2007, to notify ELEC that he or she is seeking to qualify to receive Clean Elections funds in the 2007 general election. This form must be filed before a candidate may accept any seed money or qualifying contributions.
 

Certificate of Organization and Designation of Campaign Treasurer and Depository

  A candidate must file the form D-1 to designate a campaign treasurer and to report each account established by the candidate committee. Separate accounts must be maintained for the deposit of seed money contributions and for the deposit of qualifying contributions and grant monies received from the fund. The Form D-1 must be filed no later than 10 days after establishing a candidate committee for the 2007 general election.

Seed Money Contribution Form
  A form must be completed for each seed money contribution received. The form shall be maintained by the candidate committee and shall be provided to the Commission upon request.
     
Qualifying Contribution Form

  A form must be completed for each qualifying contribution received. The form shall be maintained by the candidate committee and shall be provided to the Commission upon request.

A candidate seeking certification will be required to file Forms CE and CE-S on a weekly basis by 12:00 noon beginning on May 23, 2007. A candidate by direct petition must start filing weekly starting on the 30th day after he or she files the Declaration of Intent. The reporting dates are as follows:

  • May 9 and 16, 2007(optional filing dates for a candidate who has raised 400 contributions by these dates)
  • May 23 and 30, 2007
  • June 6, 13, 20, and 27, 2007
  • July 5, 11, 18, and 25, 2007
  • August 1, 8, 15, 22, and 29, 2007
  • September 5, 12, 19, and 26, 2007
  • October 3, 2007
     
Election Cycle and Quarterly Reports

Report of Contribtuions and Expenditures
 

After a candidate has raised the maximum amount (800) of qualifying contributions or once the qualifying period is complete, a certified candidate must file the Form CE to file the 29-day preelection, 11-day preelection and 20-day postelection reports. The dates for submitting these reports shall be:

  • October 9, 2007
    29-day preelection report
  • October 26, 2007
    11-day preelection report
  • November 26, 2007
    20-day postelection report
  • April 15, 2008
    (a final postelection quarterly report is required to be filed if the 20-day postelection report was not a final report)

Note: A certified candidate is required to return any unspent funds to the Commission by March 31, 2008.

     

Candidate Ballot Statement

Candidate Ballot Statement

  A candidate certified no later than August 17, 2007 may submit a statement (not to exceed 250 words) to the Commission to be mailed with sample ballots to each registered voter in the district in which the candidate is seeking office.

Claim for Additional Funds for Excess Contributions

  A candidate may claim funds in addition to the grant amount if a noncertified candidate in the same legislative district reports contributions in excess of the grant amount.
     

Claim for Additional Funds for Independent Expenditures
(Form CE-4)
Claim for Additional Funds for Independent Expenditures

  A candidate may claim funds in addition to the grant amount if a non-certified candidate in the same legislative district is benefiting from an independent expenditure or if the certified candidate is the subject of unfavorable publicity paid for by an entity not acting in concert with the non-certified candidate.
     

Debate Certification

Debate Certification

  Each Clean Elections candidate must file Form CE-6 no later than October 3, 2007 or no later than 5 days before the first debate occurs, whichever date occurs first; to report the arrangements, including dates and sponsor organizations, for the two mandatory Clean Elections candidates’ debates.
