Form Description
D-1 The “Single Candidate Committee Certificate of Organization and Designation of Campaign Treasurer and Depository” (Form D-1) must be filed no later than ten days after establishing a candidate committee.
D-2 The “Joint Candidates Committee Certificate of Organization and Designation of Campaign Treasurer and Depository” (Form D-2) must be filed no later than ten days after establishing a joint candidates committee.
DX The “Designation of Deputy Treasurer and/or Additional Depository” (Form DX) is filed no later than five days after the appointment of a deputy treasurer or the designation of an additional depository.
PC The “Political Committee Registration Statement and Designation of Campaign Treasurer and Depository” (Form PC) must be filed no later than 10 days after a political committee is established.
RC-1 The “Recall or Recall Defense Committee Certificate of Organization and Designation of Campaign Treasurer and Depository” (Form RC-1) must be filed no later than 10 days after a recall or recall defense committee is established.
RC-2 The “Recall Committee Registration Statement” (Form RC-2) must be filed no later than 10 days after a recall committee is established.
COM Indicates that the candidate is part of a joint candidates committee.
A-1 The “Candidate Sworn Statement” (Form A-1) is a one-time filing due on or before the 29-day preelection filing date. It is filed in lieu of the long form and can be filed by any candidate committee expending $3500 or less, including nothing.
A-2 The “Joint Candidates Committee Sworn Statement” (Form A-2) is a one-time filing due on or before the 29-day preelection report due date. It is filed in lieu of the long form when the joint candidate committee (of 2 candidates) expends $6700 or less or (committee of 3 or more candidates) expends $9700 or less, including nothing.
A-4 The “Recall Committee Sworn Statement” (Form A-4) is a one-time filing due on or before the 29-day preelection filing date of the Recall election.
C-1 or C1-Q The “Supplemental Contributor Information” (Form C-1) is filed by those candidates/committees qualifying to file the Forms A-1, A-2, or A-4 who or which receive contributions in excess of $300 in the aggregate from one source.
R-1 The “Report of Contributions and Expenditures” (Form R-1) must be filed 29 and 11 days before the date of the election, and 20 days after the date of the election.

R1-Q, RQ-2004, RQ-2005 etc.

If a committee exists more than five months prior to the date of the 29-day preelection report, the Form R-1 is filed at quarterly intervals. The “RQ” indicates that it is a quarterly report.
48-HR 48-Hour Notices of Contributions. All candidates and committees, regardless of the amount spent, are required to file written notice with the Commission within 48 hours of the receipt of any contribution in excess of $1000 in the aggregate, from one source, received during the period starting with the 13th day prior to the election up to and including the date of the election of any such contribution. This notice may be filed by using Form C-1, a letter, a mailgram, a telegram, or any other written means.
48-HR 48-Hour Notices of Expenditures. A political committee which makes expenditures in excess of $1000 in the aggregate starting with the 13th day prior to an election up to and including the day of the election must file a notice with the Commission within 48 hours of making the expenditure. This 48-hour notice may be filed by using the Form E-1, a letter, telegram, mailgram, or any other written means.
