Business Entity Annual Statement (Form BE) and Instructions
ALERT: READ BEFORE YOU DOWNLOAD THE FORM
ADOBE ACROBAT READER IS THE ONLY PROGRAM PERMISSIBLE TO OPEN AND VIEW THE FORM BE. Although other software programs such as Foxit Pro and Nitro will allow you to fill-in the form, their use will corrupt the form and make it unable to be processed by the Commission's system. Unfortunately, when a form has already been opened by one of these other programs it will need to be discarded and a new FORM BE downloaded and opened in Adobe Reader. Adobe Reader is FREE. Download it HERE 
To make sure you are using the correct program, after opening the form, the top left of the form will show the form name followed by either Adobe Reader or Adobe Acrobat Pro (see images below)

Avoid Internet congestion and possible delays. Due to the large number of expected filers, the Commission suggests that you file your report early. Statements filed for calendar years 2006-2016 are available for public inspection under Search Information .
